Background:

Subependymomas are rare, benign intraventricular tumors, typically asymptomatic and discovered incidentally.
Symptomatic cases requiring surgical intervention are exceptionally uncommon.

Observation:

A 55-year-old male presented with loss of consciousness and headaches.
MRI revealed multiple intraventricular masses causing obstructive hydrocephalus and midline shift.
Surgical resection via right frontal craniotomy achieved total tumor removal.

Findings:

This case presents multifocal supratentorial subependymomas causing obstructive hydrocephalus in a symptomatic patient.
This is only the second reported instance of multifocal subependymomas in a symptomatic individual.
Complete surgical excision resulted in significant symptomatic improvement.

Implications:

Highlights the potential for rare tumor presentations to cause significant neurological deficits.
Emphasizes the importance of surgical management for symptomatic intraventricular tumors.
