  • Blepharospasm involves involuntary eyelid closure due to muscle cocontraction.
  • Apraxia of eyelid opening is an inability to initiate eyelid opening.
  • Blepharospasm and apraxia of eyelid opening may precede Parkinson's disease development.

Purpose of the Study:

  • To investigate dopaminergic dysfunction in patients with blepharospasm or apraxia of eyelid opening.
  • To identify early signs of parkinsonism in these patient groups.

Main Methods:

  • 24 patients with blepharospasm (with or without apraxia of eyelid opening) and no parkinsonism signs underwent 123I-FP-CIT (DAT) SPECT.
  • SPECT imaging assessed presynaptic dopaminergic transporter availability.

Main Results:

  • Dopamine transporter (DAT)-SPECT was abnormal in 11 (46%) patients.
  • Abnormalities were observed in both isolated blepharospasm and blepharospasm with apraxia of eyelid opening groups.
  • The mean disease duration in affected patients was 11 years.

Conclusions:

  • Presynaptic dopaminergic dysfunction is present in nearly half of blepharospasm patients.
  • Blepharospasm, with or without apraxia of eyelid opening, may serve as an early clinical sign of parkinsonian syndromes.
