To explore the role of nursing leadership in transforming healthcare service delivery.
To examine the impact of the St. Joseph's Health System Integrated Comprehensive Care (ICC) Program.
To highlight how integrated care models improve patient outcomes and system efficiency.

Main Methods:

Case study analysis of the St. Joseph's Health System (SJHS) Integrated Comprehensive Care (ICC) Program.
Examination of bundled care and funding models across the hospital-to-home continuum.
