A new reflectance-based projection-resolved (rbPR) Optical Coherence Tomography Angiography (OCTA) method significantly improves visualization of deep retinal and choroidal vasculature by reducing projection artifacts. This technique enhances vascular integrity and detail in both healthy and diseased eyes.

Area of Science:

  • Ophthalmology
  • Medical Imaging
  • Biomedical Engineering

Background:

  • Optical coherence tomography angiography (OCTA) suffers from projection artifacts, obscuring deeper vascular layers.
  • Existing projection-resolved (PR) OCTA improves depth resolution but compromises vascular integrity in deeper plexuses.

Purpose of the Study:

  • To introduce a novel reflectance-based projection-resolved (rbPR) OCTA algorithm.
  • To enhance 3D OCTA by improving flow signal and suppressing projection artifacts using OCT reflectance.

Main Methods:

  • Developed a novel reflectance-based projection-resolved (rbPR) OCTA algorithm.
  • Quantitatively assessed vascular connectivity and plexus discrimination in healthy eyes.
  • Qualitatively evaluated artifact removal and vascular integrity in age-related macular degeneration and diabetic retinopathy models.

Main Results:

  • rbPR OCTA significantly improved vascular connectivity and discrimination of deeper plexuses in healthy eyes compared to PR-OCTA.
  • rbPR OCTA more effectively removed flow projection artifacts from the outer retinal slab in age-related macular degeneration.
  • Vascular integrity of intermediate and deep capillary plexuses was preserved in diabetic retinopathy using rbPR OCTA, with improved choriocapillaris resolution.

Conclusions:

  • The novel rbPR OCTA algorithm effectively suppresses projection artifacts while preserving and enhancing vascular integrity in deeper retinal and choroidal layers.
  • rbPR OCTA offers superior performance for visualizing microvasculature in various ocular conditions, achieving details comparable to scanning electron microscopy.
