Background:

Severe erectile dysfunction (ED) affects a significant population, with various underlying causes.
Pharmacological treatments for ED are not always effective or tolerated.
Prosthetic surgery is an option for severe ED, but less invasive methods are preferred when feasible.

Purpose of the Study:

To evaluate the efficacy of Alprostadil cream in conjunction with a vacuum device for managing severe erectile dysfunction (ED).
To assess the feasibility and patient satisfaction with this combined treatment approach prior to considering surgical interventions.

Main Methods:

A study involving 12 male patients (aged 55-65) with severe ED and no cavernous fibrosis.
Patients self-administered 3 mg of Alprostadil cream into the urethral meatus.
A vacuum device was applied over the penis after cream application.

Main Results:

Alprostadil cream facilitated rapid erection with sufficient rigidity for vacuum device application.
Sufficient erection was maintained with a base ring to enable penetration.
Home use of the Alprostadil cream and vacuum device combination proved reproducible and satisfactory for patients.

Conclusions:

The combination of Alprostadil cream and vacuum device is an effective preliminary treatment for severe ED.
This method provides a satisfactory and reproducible option for achieving intercourse in patients with ED.
It represents a valuable alternative before resorting to more invasive treatments like penile implants.
