For moderate hyperdeviations due to fourth nerve palsy, 1-muscle and 2-muscle surgeries showed similar motor and diplopia success rates. Two-muscle surgery resulted in more overcorrections, while one-muscle surgery led to more undercorrections.

- No clear advantage of two-muscle surgery was identified for motor outcomes in this patient group.
