Summary

The incidence of unsuspected endometrial sarcoma (ESS) after uterine fibroid surgery was 1 in 2269. Overall uterine sarcoma incidence was 7 in 2275 for patients with presumed fibroids or suspected sarcoma.

Background:

  • Uterine fibroids are common, but the risk of unsuspected uterine sarcoma during myomectomy or hysterectomy requires evaluation.
  • Distinguishing benign fibroids from uterine sarcoma preoperatively can be challenging.

Purpose of the Study:

  • To determine the incidence of unsuspected uterine sarcoma in patients undergoing surgery for benign uterine fibroids.
  • To assess the overall incidence of uterine sarcoma in patients with presumed uterine fibroids or suspected malignancy before surgery.

Main Methods:

  • Retrospective study at a single university hospital from 2003-2015.
  • Collected and analyzed data from patient records for uterine fibroids and uterine sarcoma.
  • Utilized descriptive statistics and exact Clopper-Pearson intervals for confidence limits.

Main Results:

  • Seven uterine sarcomas were identified among 2269 women with uterine fibroids (incidence 7/2275).
  • Types included 4 uterine leiomyosarcomas (ULMS), 1 endometrial sarcoma (ESS), 1 high-grade undifferentiated uterine sarcoma (HGUS), and 1 embryonal rhabdomyosarcoma (ERMS).
  • One case of ESS was unexpectedly found after surgery for presumed benign fibroids.

Conclusions:

  • The incidence of unsuspected endometrial sarcoma (ESS) following myomectomy or hysterectomy for benign fibroids was 1/2269.
  • The overall incidence of uterine sarcoma in patients with presumed fibroids or suspicion of malignancy was 7/2275.
