Purpose of the Study:

  • To identify clinico-pathological factors predicting renal global function after PN.
  • To investigate the role of renal volume and histopathological parameters in non-neoplastic parenchyma.

Main Methods:

  • 52 patients undergoing PN had pre- and 6-month post-operative estimated glomerular filtration rate (eGFR) and CT scans.
  • Renal parenchymal volume was assessed using 3D imaging (SYNAPSE VINCENT).
  • Multivariate logistic regression analyzed factors including glomerulosclerosis (GS) extent, renal volume, demographics, and surgical data.

Main Results:

  • Patients with >15% eGFR decline (Group B) had lower pre-operative eGFR, higher global GS extent (>10%), male gender, and proteinuria compared to Group A (<15% decline).
  • Renal volume changes were not statistically significant predictors.
  • Global GS extent (>10%) in non-neoplastic parenchyma was the sole independent predictor of significant eGFR decline.

Conclusions:

  • Host factors, particularly glomerulosclerosis extent in non-neoplastic parenchyma, are more predictive of post-PN renal function than surgical factors.
  • Global GS extent serves as a promising biomarker for predicting renal function after partial nephrectomy.
