Purpose of the Study:

  • To identify evidence gaps and set priorities for new systematic reviews and randomized controlled trials (RCTs) in the management of DR, including DME.
  • To determine the clinical importance of various research questions related to DR and DME interventions.

Main Methods:

  • A 2-round Delphi survey was administered to 410 Diabetic Retinopathy Clinical Research Network (DRCR.net) investigators.
  • Investigators rated 91 clinical research questions on a scale of 0-10 for importance and suggested additional questions.
  • High-priority questions were defined as those receiving an importance rating of 5 or higher from at least 75% of respondents in both rounds.

Main Results:

  • Twenty-two of 106 clinical research questions were identified as high priority.
  • Nine high-priority questions focused on anti-VEGF therapy effectiveness, and 13 addressed follow-up frequency and treatment effectiveness in specific patient groups.

  • Only one prioritized question was supported by conclusive evidence from a Cochrane systematic review.

Conclusions:

  • A significant number of high-priority research questions for DR and DME management lack sufficient evidence.
  • The findings underscore the necessity for conducting new systematic reviews and RCTs to address these evidence gaps.
  • Prioritizing CER in ophthalmology can lead to more relevant and impactful research outcomes.
