Background:

  • Syphilitic uveitis (SU) is an ocular manifestation of syphilis, a complex infectious disease.
  • SU can lead to significant visual impairment if not treated effectively.
  • Predictors of treatment success in SU require further elucidation to optimize patient outcomes.

Purpose of the Study:

  • To identify key predictors of successful treatment outcomes in patients with syphilitic uveitis.
  • To analyze factors influencing recovery from ocular inflammation in SU.

Main Methods:

  • A retrospective multicentric analysis was conducted on 66 patients (95 eyes) diagnosed with syphilitic uveitis.
  • Patients included those with and without human immunodeficiency virus (HIV) co-infection.
  • Treatment success was evaluated based on inflammatory parameter reduction and visual acuity at various follow-up intervals.

Main Results:

  • Recovery was achieved in 65% of eyes at 1 month and 85% at last follow-up.
  • Early clinical improvement at 1 week post-treatment was a significant predictor of recovery (cRR, 3.5).
  • Periocular dexamethasone injections and methylprednisolone pulses were associated with negative outcomes.

Conclusions:

  • Early clinical improvement is the most robust predictor of ophthalmological recovery in syphilitic uveitis.
  • Treatment strategies should consider the potential adverse effects of certain corticosteroid regimens on SU outcomes.
