Summary

Transbronchial lung cryobiopsy (TBC) for interstitial lung disease (ILD) can cause pneumothorax, especially when pleura is present in samples. Increased BMI is linked to bleeding complications.

Background:

  • Transbronchial lung cryobiopsy (TBC) is a less invasive alternative to surgical lung biopsy for diagnosing interstitial lung disease (ILD).
  • While TBC offers advantages, potential complications require thorough investigation.

Purpose of the Study:

  • To evaluate the complications associated with TBC in ILD patients.
  • To identify factors contributing to TBC-related complications.

Main Methods:

  • Prospective evaluation of 90 ILD patients undergoing TBC.
  • Analysis of clinicodemographic variables and factors influencing TBC complications using logistic regression.

Main Results:

  • Pneumothorax occurred in 22 patients, with 81.8% requiring chest tube drainage.
  • Bleeding (Grade 2-3) was observed in 14.4% of cases.
  • Visceral pleura in samples significantly increased pneumothorax risk (OR=9.59); higher BMI correlated with bleeding (OR=1.16).

Conclusions:

  • Pneumothorax is the most common TBC complication but is generally reversible.
  • Presence of pleura in biopsy samples and increased BMI are significant risk factors for pneumothorax and bleeding, respectively.
  • Further research is needed to optimize patient selection for TBC.
