Summary

Flexible endoscopic evaluation of swallowing (FEES) can worsen aspiration and residue compared to videofluoroscopy alone. This suggests FEES may overestimate swallowing impairment in dysphagia patients.

Background:

  • Aspiration pneumonia is a significant cause of mortality in the elderly population.
  • Accurate evaluation of swallowing function is crucial for managing dysphagia and preventing aspiration.
  • Videofluoroscopy (VFSS) and flexible endoscopic evaluation of swallowing (FEES) are common methods for assessing swallowing, but FEES may not capture all dynamics.

Purpose of the Study:

  • To investigate the impact of endoscope insertion during FEES on swallowing dynamics.
  • To compare swallowing function assessment using VFSS with and without an inserted endoscope.

Main Methods:

  • Thirty-seven patients with dysphagia underwent randomized VFSS with 10 cc contrast material, both with and without an inserted endoscope.
  • Key metrics evaluated included laryngeal elevation delay time, Penetration-Aspiration Scale (PAS), and Pharyngeal Residue Severity Rating Scale (PRSRT).

Main Results:

  • Laryngeal elevation delay time was similar between the two conditions (P=0.29).
  • However, both PAS (P<0.001) and PRSRT (P<0.001) scores were significantly higher with the endoscope inserted, indicating more severe aspiration and residue.
  • Patients showing no aspiration without the endoscope exhibited aspiration with it, and vice versa.

Conclusions:

  • Flexible endoscopic insertion during VFSS can lead to more severe aspiration and pharyngeal residue compared to VFSS alone.
  • FEES may overestimate the severity of swallowing impairment, potentially affecting clinical decision-making.
  • Further research is needed to understand the clinical implications of these findings in dysphagia management.
