Psychiatric disorders show significant ethnic variation in Nepal. Historically excluded Dalit groups experienced higher rates of most disorders, while Janajatis had lower PTSD rates compared to the Brahmin/Chhetri majority.
Area of Science:
- Global Mental Health
- Psychiatric Epidemiology
- Sociology of Health
Background:
- Psychiatric disorders are a leading cause of global disability.
- Ethnic variations in mental health disorders are understudied, particularly in South Asia.
- Understanding these variations is crucial for targeted public health interventions.
Purpose of the Study:
- To investigate ethnic differences in the prevalence of specific psychiatric disorders in Nepal.
- To examine variations in Major Depressive Episode (MDE), Major Depressive Disorder (MDD), Panic Attack, Panic Disorder, Post-Traumatic Stress Disorder (PTSD), and Intermittent Explosive Disorder (IED).
Main Methods:
- Utilized data from 400 adult interviews in Nepal using a controlled comparison design.
- Employed multilevel logistic regression models to analyze ethnic group differences.
- Used clinically validated World Mental Health survey instruments for diagnosis.
Main Results:
- Dalit populations exhibited significantly higher odds for most psychiatric disorders and episodes compared to the Brahmin/Chhetri group.
- Janajati populations showed significantly lower odds of being diagnosed with PTSD relative to the Brahmin/Chhetri group.
- No significant gender differences were observed for MDD or MDE.
Conclusions:
- Psychiatric disorders and episodes demonstrate significant ethnic variation within a rural South Asian population.
- Ethnicity is a significant factor influencing mental health outcomes in Nepal.
- Gender differences in the prevalence of depression were minimal in this study population.
