Background:

  • Loud music exposure is a common cause of acute acoustic trauma in adolescents.

Purpose of the Study:

  • To analyze clinical and psychophysical data of patients with music-induced hearing disorder (MIHD).
  • To construct individual hearing and tinnitus profiles.
  • To investigate the underlying pathology of music-induced hearing disorders.

Main Methods:

  • Analysis of clinical and psychophysical data from 104 consecutive patients with MIHD.
  • Tinnitus was the primary presenting symptom in all cases.
  • Evaluation included pure tone audiometry, Tinnitus Handicap Inventory (THI), and Visual Analog Scales (VAS).

Main Results:

  • Two-thirds of tinnitus patients had normal hearing by conventional audiometry.
  • High-frequency tinnitus was reported by 83% of patients.
  • Hyperacusis was present in 65%, and 71% reported sleep disturbances. VAS scores correlated strongly with THI scores.

Conclusions:

  • Music-induced acute acoustic trauma is not always detected by conventional audiometry.
  • Results suggest 'silent hearing loss' with afferent nerve terminal damage as the pathology.
  • Tinnitus and hyperacusis are common sequelae, indicating a need for advanced diagnostic approaches.
