Summary

Current criteria for severe aplastic anaemia are inadequate. Lowering the reticulocyte count threshold is essential for accurate diagnosis and improved patient prognosis in aplastic anaemia cases.

Background:

Severe aplastic anaemia (SAA) is a rare but life-threatening condition.
Accurate diagnostic criteria are crucial for timely and appropriate treatment initiation.
Current international criteria for SAA may require re-evaluation.

Purpose of the Study:

To assess the validity of existing international criteria for diagnosing severe aplastic anaemia.
To determine if current peripheral blood value thresholds accurately reflect disease severity.

Main Methods:

Analysis of 213 consecutive bone marrow aplasia cases from a single institution.
Evaluation of peripheral blood values, including reticulocyte counts corrected for haematocrit.
Application of multivariate analysis to identify significant predictors of disease severity.

Main Results:

The current reticulocyte count limit of 1% (corrected for haematocrit) is insufficient for identifying severe aplastic anaemia.
Distribution percentiles indicate that a lower reticulocyte threshold is needed.
Multivariate analysis supports the inadequacy of the current reticulocyte count criterion.

Conclusions:

The established international criteria for severe aplastic anaemia require modification.
A substantially lowered reticulocyte count threshold is recommended for improved diagnostic accuracy.
Revised criteria will aid in better patient stratification and treatment decisions for aplastic anaemia.
