Background:

  • Borderline personality disorder (BPD) is characterized by emotional instability and relationship difficulties.
  • Abnormal brain anatomy is suspected in BPD, but white matter microstructure and its relation to impulsivity remain unclear.
  • This study investigates BPD's link to diffusion tensor imaging (DTI) parameters and psychological assessments.

Purpose of the Study:

  • To explore the relationship between BPD and DTI parameters.
  • To examine correlations between white matter microstructure and psychological test results in BPD patients.

Main Methods:

  • Diffusion tensor imaging (DTI) tractography was performed on 35 unmedicated BPD patients and 50 healthy controls (HCs).
  • Participants completed psychological tests including the Childhood Trauma Questionnaire (CTQ), Profile of Mood State (POMS), and Depression and Anxiety Cognition Scale (DACS).

Main Results:

  • BPD patients showed significantly lower axial diffusivity (AD) in the cingulum and front-occipital/longitudinal fasciculi compared to HCs.
  • Reduced AD in the cingulum correlated with higher depression scores (POMS).
  • Reduced AD in the inferior front-occipital fasciculus correlated with future denial (DACS).

Conclusions:

  • Findings suggest front-limbic and front-occipital network dysfunction in BPD, characterized by cingulum and inferior longitudinal fasciculus alterations.
  • These microstructural differences may relate to mood and cognitive impairments in BPD.
  • Further research with larger, diverse samples is needed to confirm these results.
