Acute Exudative Polymorphous Paraneoplastic Vitelliform Maculopathy Managed With Intravitreal Aflibercept

• Acute exudative polymorphous paraneoplastic vitelliform maculopathy (AEPPVM) is a rare condition often associated with underlying malignancies.

• Two patients with bilateral AEPPVM, secondary to treated breast and colon carcinoma respectively, were treated with intravitreal aflibercept.
• Enhanced depth imaging optical coherence tomography (EDI-OCT) revealed characteristic findings including sensorineural retinal detachment and subretinal material.

Findings:

• Intravitreal aflibercept injections led to improved visual acuity in both patients.
• EDI-OCT demonstrated a reduction in subretinal vitelliform material and intraretinal edema following treatment.

Implications:

• Intravitreal aflibercept may be an effective treatment for controlling the progression of AEPPVM.
• This therapeutic approach could stabilize retinal pigment epithelial function and reduce vascular leakage in patients with AEPPVM.
