The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. Background:

  • Ankle sprains are common injuries managed conservatively when fractures are absent.
  • Current methods lack markers to identify patients at risk for persistent instability or disability.
  • Predicting long-term functional outcomes after ankle sprains remains a clinical challenge.

Purpose of the Study:

  • To analyze the characteristics of soft tissue ankle injuries.
  • To determine if the anatomical severity of an ankle injury predicts its functional outcome.
  • To identify potential markers for predicting recovery after acute ankle injuries.

Main Methods:

  • 100 patients with acute ankle injuries (no radiographic fracture) were recruited.
  • Diagnostic ultrasound, cone beam CT, and disability assessments were performed within 5 days.
  • Ultrasound and physiotherapy assessments were repeated at 3 and 6 months post-injury.

Main Results:

  • Patients were categorized into simple (58), complex (21), and fracture (19) groups.
  • No significant differences in pain, disability, or functional outcomes were observed between injury groups at any study point.
  • Individual recovery trajectories varied, irrespective of initial injury classification.

Conclusions:

  • Anatomical severity of ankle injuries, based on imaging, does not solely determine medium to long-term clinical outcomes.
  • Some patients with simple ankle injuries experienced ongoing problems, while others with complex injuries recovered fully.
  • Radiological assessment alone is insufficient for predicting symptom severity or recovery trajectory in ankle sprains.
