Background:

  • Extra-adrenal myelolipomas are rare tumors with limited documented cases.

  • Three cases of extra-adrenal myelolipomas were identified in elderly women (68-83 years).
  • Lesions were presacral, encapsulated, and varied in size (6-12 cm) and weight (72-330 g).
  • Macroscopically, they presented with a yellow to red-brown variegated appearance.

  • Microscopic examination revealed mature adipose tissue and normal hematopoietic elements.
  • The origin of these tumors remains unclear, with choristomatous origin being a favored theory.
  • Distinguishing these from extramedullary hematopoietic tumors is essential.

  • This case series expands the literature on extra-adrenal myelolipomas.
  • Accurate differentiation from other hematopoietic tumors is critical for patient management.
  • Further research into the etiology of these rare lesions is warranted.
