Summary

Electrical impedance spikes in cochlear implants may signal inner ear pathology, predicting residual hearing loss. Monitoring these impedance changes could help manage hearing preservation after cochlear implant surgery.

Background:

  • Cochlear implant surgery increasingly aims to preserve residual low-frequency hearing.
  • Delayed loss of residual hearing remains a significant challenge post-implantation.

Purpose of the Study:

  • To investigate if electrical impedance fluctuations in cochlear implant electrodes can serve as a biomarker for pathological changes leading to delayed residual hearing loss.

Main Methods:

  • Secondary analysis of a double-blinded randomized trial involving methylprednisolone administration before cochlear implantation.
  • Investigated 74 patients for impedance "spikes" (≥4µΩ median rise) and correlated them with hearing loss, dizziness, and tinnitus.

Main Results:

  • An impedance spike occurred in 14% of patients, strongly correlating with subsequent residual hearing loss.
  • At 1 year, 90% of patients with spikes lost all residual hearing, compared to 8.1% without spikes.
  • Seventy percent of patients with spikes also experienced vertigo; steroid administration did not affect spike occurrence.

Conclusions:

  • A relationship exists between impedance spikes and the loss of residual hearing.
  • Impedance rises appear to reflect inner ear pathology and can predict future residual hearing loss.
