Summary

Surgical resident experience in standard laparoscopy partially translates to single-incision laparoscopy (SIL) skills. Dedicated SIL training is recommended to further enhance proficiency in these advanced laparoscopic techniques.

Background:

  • The study investigated skill translation across surgical postgraduate year (PGY) levels.

Purpose of the Study:

  • To compare surgical residents' psychomotor skills in SIL across all PGY levels.
  • To determine if experience in standard laparoscopy improves SIL proficiency.

Main Methods:

  • 31 surgical residents (PGY 1-5) performed SIL tasks on a simulator.
  • Tasks included bowel manipulation, endoloop placement, and suture tying (extracorporeal and intracorporeal).
  • Performance was assessed by a certified proctor based on task completion within 5 minutes.

Main Results:

  • Minimal prior SIL exposure was reported across all resident levels.
  • PGY level significantly correlated with improved task completion (P=0.005).
  • Higher PGY levels showed better performance on simpler tasks, but difficulty impacted success rates (e.g., PGY5 100% bowel run vs. 0% intracorporeal knot tying).

Conclusions:

  • Some psychomotor skills appear transferable from standard to single-incision laparoscopy.
  • Dedicated training in SIL techniques is likely necessary for optimal skill development.
