This study found that while most patients don't misuse prescription cannabinoids (CBs), those with psychiatric issues or a history of substance abuse are at higher risk. Routine monitoring for problematic cannabinoid use is recommended.
Area of Science:
- Medical research
- Pharmacology
- Clinical practice
Background:
- Cannabinoids (CBs) show therapeutic benefits for various medical conditions.
- Concerns exist regarding the potential misuse and problematic use of CBs.
Purpose of the Study:
- To determine the incidence of problematic prescription cannabinoid use (PPCBU) in patients starting cannabinoid therapy.
- To identify factors associated with PPCBU.
Main Methods:
- Prospective observational study of 265 patients prescribed oral cannabinoids.
- Baseline assessments included demographics, clinical data, and substance use.
- Problematic prescription cannabinoid use (PPCBU) was assessed at 3, 6, and 12 months using three validated measures.
Main Results:
- The majority of patients scored below cutoff for PPCBU at all follow-up points (p < .001).
- A maximum of approximately 25% of patients exhibited PPCBU at any given time point.
- History of psychiatric problems, tobacco smoking, and recreational cannabis use were associated with higher odds of PPCBU (p < .05).
- Past-year substance abuse (DAST-20) was the strongest predictor of PPCBU (p < .005).
Conclusions:
- While most patients do not develop problematic prescription cannabinoid use (PPCBU), clinicians should routinely monitor for it.
- Particular vigilance is advised for patients with a history of psychiatric or substance use disorders.
- These findings inform clinical practice regarding the safe management of cannabinoid therapy.
