Purpose of the Study:

  • To identify the prevalence, clinical profile, resource utilization, and outcomes of tropical fevers in Indian ICUs.
  • To inform diagnostic and therapeutic strategies for tropical fevers.

Main Methods:

  • A multicenter prospective observational study involving 34 ICUs across India.
  • Enrolled critically ill adults and children with nonlocalizing fever and signs of organ dysfunction.

Main Results:

  • Dengue, scrub typhus, encephalitis, and malaria were the most common etiologies.
  • High rates of mechanical ventilation, vasoactive therapy, and renal replacement therapy were observed.
  • Mortality was 18.4%, with higher rates in patients with undiagnosed etiology.

Conclusions:

  • Dengue, scrub typhus, encephalitis, and malaria are leading causes of tropical fevers in Indian ICUs.
  • A syndromic approach, point-of-care tests, and empiric antimicrobial therapy are recommended.
