Proteins are chains of amino acids linked together by peptide bonds. Upon synthesis, a protein folds into a three-dimensional conformation, critical to its biological function. The native conformation of a protein is formed by interactions between the side chains of its constituent amino acids. When the amino acids cannot form these interactions, the protein cannot fold by itself and needs chaperones. The apparent volume of distribution (Vd) is a crucial pharmacokinetic parameter representing the hypothetical body fluid volume into which a drug disperses. It is calculated based on the total amount of drug in the body (estimated from the administered dose and bioavailability) divided by the plasma drug concentration. The total amount of drug in the body does not directly refer to the dose given but is derived by accounting for absorption, distribution, metabolism, and excretion processes.

Summary

Cheek fillers do not lift the skin or elevate the nasolabial fold. Improvement in the nasolabial fold is perceived when the nasojugal crease is expanded, not from cheek filler alone.

Background:

  • Common belief suggests cheek fillers lift skin and improve nasolabial folds.
  • This perceived effect of cheek augmentation remains scientifically unsubstantiated.

Purpose of the Study:

  • To investigate the biomechanical effects of hyaluronic acid cheek fillers on facial skin.
  • To determine if cheek injections cause longitudinal skin shortening or elevation of the nasolabial crease.

Main Methods:

  • Utilized a 3D camera system to evaluate 77 patients before and after cheek filler injections.
  • Analyzed a subgroup of 37 patients with consistent facial expressions to assess skin traction.

Main Results:

  • Observed skin expansion perpendicular to the injection site, not longitudinal shortening.
  • No lateral traction or elevation of the nasolabial crease was detected from cheek injections.
  • Improvement in the medial face was only seen when filler was placed in the nasojugal crease.

Conclusions:

  • Cheek filling with 3 cc of hyaluronic acid does not create traction forces on the skin towards the nasolabial crease.
  • The visual improvement of the nasolabial fold is likely due to the expansion of the nasojugal crease, not direct skin lifting from cheek fillers.
