    Background:

    • Acromioclavicular (AC) joint dislocations, particularly Rockwood type III and higher, present significant challenges in shoulder function.
    • Surgical interventions aim to restore stability and optimize functional recovery, but optimal techniques remain debated.

    Purpose of the Study:

    • To compare the clinical outcomes of Clavicular Hook Plate Fixation versus the EndoButton technique for Rockwood type III (or above) AC joint dislocations.
    • To identify factors influencing postoperative functional recovery in these patients.

    Main Methods:

    • A prospective study involving 60 patients with Rockwood type III (or above) AC dislocations.
    • Patients were randomized into two groups: Clavicular Hook Plate Fixation (Group A) and EndoButton technique (Group B).
    • Functional recovery was assessed using the Constant shoulder score pre- and post-surgery, with statistical analysis of influencing factors.

    Main Results:

    • Both surgical techniques significantly improved Constant shoulder scores post-operation (P < 0.05).
    • The EndoButton technique group (Group B) demonstrated significantly better scores at six months compared to the Hook Plate group (Group A) (P < 0.05).
    • No significant difference was observed between groups at fifteen months. Patient compliance with rehabilitation guidance strongly correlated with improved Constant scores (P < 0.05).

    Conclusions:

    • Both Clavicular Hook Plate and EndoButton techniques are effective for treating severe AC joint dislocations.
    • The EndoButton technique shows an advantage in promoting earlier functional recovery.
    • Adherence to postoperative rehabilitation is a critical determinant of successful functional outcomes.
