Summary

This case report highlights a rare colonic leiomyosarcoma (LMS) mimicking a gastrointestinal stromal tumor (GIST). Accurate diagnosis via immunohistochemistry is crucial for appropriate management of this uncommon soft tissue sarcoma.

Background:

  • Leiomyosarcoma (LMS) is a prevalent soft tissue sarcoma, yet primary colonic LMS is exceptionally rare, comprising only 1-2% of gastrointestinal malignancies.
  • Soft tissue sarcomas require precise histological and immunohistochemical characterization for accurate diagnosis and treatment planning.

Observation:

  • A 55-year-old female presented with acute right lower quadrant abdominal pain, initially investigated with colonoscopy and CT scan.
  • Imaging revealed a colonic mass suggestive of a gastrointestinal stromal tumor (GIST), necessitating further diagnostic evaluation.

Findings:

  • Histopathological examination and immunohistochemical profiling, including smooth muscle actin, desmin, and CD34, confirmed the diagnosis of colonic LMS.
  • The patient underwent successful surgical resection (right hemicolectomy) with no adjuvant oncological treatment and showed no recurrence at 5 months post-surgery.
