    Summary

    Survivors of suicide attempts involving handguns can retain some neurological function despite bilateral optic nerve destruction. This case series highlights the potential for limited additional neurological damage in such severe injuries.

    Background:

    • Bilateral optic nerve destruction is typically considered a catastrophic injury with severe visual impairment.
    • Suicide attempts using firearms present unique challenges in trauma management and outcome prediction.

    Observation:

    • Four patients who attempted suicide via handgun survived injuries resulting in bilateral optic nerve destruction.
    • These patients experienced limited additional neurological deficits beyond visual impairment.

    Findings:

    • The study documents the survival and neurological status of four individuals with severe optic nerve trauma from firearm injury.
    • Despite the bilateral optic nerve destruction, significant additional neurological damage was not observed in these cases.

    Implications:

    • This suggests that immediate medical intervention and management protocols may mitigate secondary neurological damage in severe optic nerve trauma.
    • Further research is warranted to understand the factors influencing neurological preservation in patients with firearm-induced optic nerve injuries.
