Background:

  • Sleep-disordered breathing (SDB) is a known risk factor for stroke and is linked to poorer outcomes.
  • Potential sex-based differences in the relationship between SDB and stroke warrant investigation.

Purpose of the Study:

  • To compare the prevalence and severity of pre-stroke and post-stroke SDB in men and women.
  • To investigate the association between menopausal status and post-stroke SDB.

Main Methods:

  • A cross-sectional study of 1815 subjects from the BASIC project.
  • Pre-stroke SDB risk assessed via the Berlin Questionnaire; post-stroke SDB evaluated using portable respiratory monitoring in 832 subjects.
  • Log Poisson and linear regression models were employed for statistical analysis, adjusting for confounders.

Main Results:

  • Women had lower risk for pre-stroke SDB (PR 0.87) and lower prevalence of post-stroke SDB (PR 0.71) compared to men.
  • Post-stroke SDB was more severe in men (mean difference in respiratory event index 6.5).
  • No significant association was found between post-stroke SDB and menopausal status or age at menopause.

Conclusions:

  • Sleep-disordered breathing is more prevalent and severe in men compared to women following acute ischemic stroke.
  • These findings highlight potential sex-specific considerations in stroke care and SDB management.
