  • 100 patients with schizophrenia or schizoaffective disorder were assessed using the Bern Psychopathology Scale (BPS) every 6 months for up to 18 months.
  • The BPS assesses language, affectivity, and motor behavior domains, rating them as normal, inhibited, or disinhibited.
  • Qualitative case reports supplemented quantitative psychopathology data.

Main Results:

  • Of 48 patients completing at least two assessments over one year, 33.3% showed bipolar course patterns in one domain, and 12.5% in multiple domains.
  • Frequent shifts between dominant symptom domains were observed (41.7%), with shifts between single and combined dominant domains being more common (68.8%).

Conclusions:

  • Schizophrenia exhibits a heterogeneous course with dynamic psychopathology changes.
  • These findings necessitate consideration in patient communication, illness mechanism research, and treatment development.
  • The study's small sample size is a significant limitation.
