Area of Science:

  • Ophthalmology
  • Oncology
  • Pathology

Background:

  • Choroidal melanoma is a rare intraocular malignancy.
  • Hemorrhagic presentation of choroidal melanoma is uncommon.

Purpose of the Study:

  • To illustrate the clinicopathologic correlation of a hemorrhagic choroidal melanoma.
  • To describe a rare case of choroidal melanoma with significant hemorrhage.

Main Methods:

  • Case report of a 52-year-old patient with a large choroidal mass and hemorrhage.
  • Enucleation of the affected eye for histopathological examination.
  • Analysis of tumor cells (MART1 positive, epithelioid type) and their relationship with Bruch's membrane.

Main Results:

  • Histopathology confirmed epithelioid-type MART1 positive choroidal melanoma.

  • Tumor rupture through Bruch's membrane was identified.
  • Vascular compression and massive hemorrhage into subretinal space, retina, and vitreous were observed.
  • Conclusions:

    • Tumor rupture through Bruch's membrane can cause a tourniquet effect, leading to massive hemorrhage in choroidal melanoma.
    • This case underscores the importance of clinical suspicion for diagnosing hemorrhagic choroidal melanoma.
    • Understanding the mechanism of hemorrhage is crucial for patient management.
