A Lymphoma Almost Overlooked.

Summary

This case highlights managing synchronous cancers in an HIV-positive patient. Effective chemotherapy achieved remission for both lymphoma and vaginal cancer, though infectious complications proved fatal.

Background:

Human immunodeficiency virus (HIV) infection is associated with an increased risk of various malignancies.
Synchronous malignancies, particularly in immunocompromised individuals, present complex diagnostic and therapeutic challenges.
Accurate staging and management require careful consideration of both cancer and potential opportunistic infections.

Observation:

A 50-year-old female with untreated HIV presented with vaginal squamous cell carcinoma and synchronous diffuse large B-cell lymphoma in the liver.
The patient received chemotherapy directed at the lymphoma.
Remarkably, both the lymphoma and the vaginal carcinoma showed complete response to the lymphoma-directed chemotherapy.

Findings:

Complete remission of diffuse large B-cell lymphoma was achieved with standard chemotherapy.

Vaginal squamous cell carcinoma also demonstrated a complete clinical response to the same lymphoma-directed therapy.

Despite successful cancer treatment, the patient succumbed to infectious complications during surgery for a vaginal fistula.

Implications:

This case underscores the importance of thorough diagnostic workup to identify synchronous malignancies and infections in HIV-positive patients.
Management strategies for HIV-associated malignancies may require tailored and sequenced chemotherapeutic regimens.
Successful treatment of one malignancy with therapy directed at another highlights potential therapeutic overlaps and the need for individualized care plans.
