Kleine-Levin syndrome (KLS) patients can develop new psychiatric disorders between episodes. This study found that nearly one in five KLS patients experienced emerging mood, anxiety, or other disorders, challenging the notion of KLS being benign between episodes.
Area of Science:
- Neurology
- Psychiatry
- Sleep Medicine
Background:
- Kleine-Levin syndrome (KLS) is characterized by recurrent episodes of hypersomnia and cognitive, psychiatric, and behavioral disturbances.
- Periods between episodes are often considered asymptomatic, but the emergence of psychiatric disorders during these times is not well-understood.
Purpose of the Study:
- To investigate the incidence and types of psychiatric disorders that develop in adolescent patients with KLS during their 'asymptomatic' periods.
- To identify factors associated with the emergence of these comorbid psychiatric conditions.
Main Methods:
- A naturalistic, uncontrolled clinical cohort study involving 115 adolescent patients diagnosed with primary KLS.
- Patients underwent annual psychiatric interviews for 1-10 years to diagnose pre-existing and new comorbid psychiatric disorders.
- Data on demographics, KLS episode characteristics, and psychiatric history were collected.
Main Results:
- 16.5% of patients had a history of psychiatric disorders before KLS onset.
- 21% of patients developed new comorbid psychiatric disorders (including mood, anxiety, eating, psychotic, and substance use disorders) 1-6 years after KLS onset, during asymptomatic periods.
- Female sex, longer disease duration, longer time incapacitated, and more frequent psychiatric symptoms during episodes were associated with developing new psychiatric disorders.
Conclusions:
- Contrary to the assumption of normality between episodes, a significant proportion of KLS patients develop psychiatric disorders during asymptomatic periods.
- The emergence of these disorders may be linked to underlying personal vulnerability and psychiatric symptoms experienced during KLS episodes.
- These findings highlight the need for ongoing psychiatric monitoring in KLS patients, even between episodes.

Bone Disorders
Bone deposition is also affected by the levels of sex hormones like estrogen and testosterone that promote osteoblast activity and bone matrix synthesis. When the level of these hormones decreases due to aging, it causes a reduction in bone deposition. As a result, bone resorption by osteoclasts...
