Summary

Exercise-induced left bundle branch block (EI-LBBB) is rare and occurs more often in women. This study found EI-LBBB did not increase the risk of major cardiovascular events in patients undergoing exercise stress tests.

Background:

  • Exercise-induced left bundle branch block (EI-LBBB) is an uncommon finding during cardiac stress testing.
  • Its clinical significance and prognostic implications remain largely unknown.

Purpose of the Study:

  • To investigate the scintigraphic characteristics of EI-LBBB.
  • To evaluate the prognostic value of EI-LBBB in patients undergoing exercise stress testing.

Main Methods:

  • Retrospective review of 1,885 patients undergoing GATED-SPECT ergometry.
  • Analysis of 7 patients who exhibited EI-LBBB during exercise testing.
  • Coronary angiography and long-term cardiovascular event monitoring (30±8 months).

Main Results:

  • EI-LBBB prevalence was 0.37%, predominantly observed in women (6/7).
  • Myocardial function and perfusion were normal in 3 patients; perfusion defects were present in 4.
  • No coronary artery disease was detected in patients with perfusion defects.

Conclusions:

  • EI-LBBB is a rare finding, more prevalent in women.
  • In this cohort, EI-LBBB was not associated with coronary artery disease or an increased risk of major cardiovascular events.
  • Non-ischaemic structural heart disease was noted in 2 patients.
