Medulloblastoma (MB) shows low PD-L1 expression, aiding immune escape. However, TH1 cytokine stimulation potently induces PD-L1, suggesting an inflamed tumor microenvironment is key for PD-1 pathway activation in this pediatric brain tumor.
Area of Science:
- Pediatric Oncology
- Immunology
- Neuro-oncology
Background:
- Medulloblastoma (MB) is the most common malignant pediatric brain tumor.
- Evaluating PD-L1 expression and immune-mediated resistance markers is crucial for understanding MB.
- This study investigates PD-L1 in human MB samples.
Purpose of the Study:
- To assess PD-L1 expression levels in human medulloblastoma.
- To identify factors influencing PD-L1 expression, including molecular subgroup and microenvironment.
- To explore the role of PD-L1 in immune escape in pediatric brain tumors.
Main Methods:
- Human MB tumor samples were analyzed for PD-L1 and immune cell markers.
- Expression analysis was correlated with molecular subgroup.
- Functional analysis of PD-L1 expression was performed under IFN-γ, radiation, and MYC overexpression conditions.
Main Results:
- Overall PD-L1 levels in human MB were low, but some cases showed focal expression with increased immune infiltrates.
- Sonic hedgehog (SHH) MB exhibited the highest PD-L1 expression.
- IFN-γ robustly stimulated PD-L1 expression in all cell lines, while radiation showed variable effects.
Conclusions:
- Medulloblastoma expresses low PD-L1, contributing to immune evasion.
- TH1 cytokine stimulation is a potent inducer of PD-L1 expression in vitro.
- An inflamed tumor microenvironment is likely necessary for PD-1 pathway activation in MB.

What is Gene Expression?
Gene expression is the process in which DNA directs the synthesis of functional products, that is, proteins. Cells can regulate gene expression at various stages. It allows organisms to generate different cell types and enables cells to adapt to internal and external factors.
