    Erythema Nodosum Leprosum (ENL) can present with rare ulcerative forms. Early diagnosis and treatment with thalidomide are crucial for effective management of severe ENL cases.

    Background:

    • Erythema Nodosum Leprosum (ENL) typically presents as evanescent, erythematous nodules.
    • Severe ENL can manifest with rare necrotic and ulcerative forms.

    Observation:

    • A 27-year-old male presented with ulcerative nodules, fever, joint pain, and pedal edema.
    • Histopathology revealed superficial dermal edema and perivascular infiltrate, with negative Fite-Faraco stain.
    • Slit smear showed granular bacilli, indicating leprosy.

    Findings:

    • The patient was diagnosed with necrotizing vasculitis, likely secondary to leprosy.
    • Treatment with oral steroids and thalidomide resulted in significant improvement.
    • Ulcers healed completely within two weeks of initiating thalidomide therapy.
