    Background:

    • Current bladder cancer (BCA) detection relies on cystoscopy, lacking sensitive and specific urinary biomarkers.
    • Urinary microRNAs (miRNAs) are emerging as promising biomarkers for early, non-invasive detection of urologic malignancies.

    Purpose of the Study:

    • To identify and validate urinary microRNAs as sensitive and specific biomarkers for non-invasive bladder cancer detection.
    • To compare urinary miRNA expression profiles between bladder cancer patients and healthy controls.

    Main Methods:

    • Expression profiling of urinary miRNAs using Affymetrix miRNA microarrays in 155 BCA patients and 83 controls.
    • Validation of candidate miRNAs using TaqMan assays and quantitative real-time polymerase chain reaction (qPCR).

    Main Results:

    • A distinct miRNA signature with significantly different urinary concentrations was identified in BCA patients versus controls (p < 0.01).
    • Three miRNAs were validated to be significantly elevated in the urine of BCA patients (p < 0.0001).
    • A significant decrease in two urinary miRNAs was observed 3 months post-surgery compared to pre-operative samples (p < 0.01).

    Conclusions:

    • Validated urinary miRNAs demonstrate significantly higher concentrations in bladder cancer patients.
    • Urinary miRNAs show potential as sensitive and specific biomarkers for the non-invasive detection of bladder cancer.
