    Area of Science:

    Background:

    • Isolated ACTH deficiency is rare in children.
    • Hypoglycemia can be a severe complication of adrenal insufficiency.

    Observation:

    • A 9-year-old girl presented with hypoglycemic coma and unresponsiveness of growth hormone (GH) to stimuli.
    • Initial evaluation revealed isolated ACTH deficiency.

    Findings:

    • Hydrocortisone replacement therapy led to normal linear growth.
    • GH response to stimulation normalized after four years of glucocorticoid treatment.
    • Cortisol deficiency is implicated as the cause of hypoglycemia and subnormal GH response.

    Implications:

    • This case suggests cortisol deficiency can impair GH secretion and cause hypoglycemia.
    • Transitory GH deficiency may occur secondary to hypophysitis.
    • Normal growth in patients with apparent anterior pituitary hormone deficiencies may be linked to cortisol's role.
