Short-distance transport refers to transport that occurs over a distance of just 2-3 cells, crossing the plasma membrane in the process. Background:

  • Rising healthcare costs necessitate a focus on high-value clinical decision-making in medical education.
  • Online Virtual Interactive Cases (VICs) were developed to simulate patient encounters, enhance price transparency, and promote cost-consciousness in rheumatology training.

Purpose of the Study:

  • To assess the effectiveness of VICs in teaching efficient resource utilization in rheumatology.
  • To evaluate trainees' diagnostic accuracy, management skills, and cost-consciousness.

Main Methods:

  • VIC modules were distributed to medical students and internal medicine residents.
  • Trainees assessed simulated patients, ordered investigations, and formulated management plans, with each action logged for time and cost.
  • Evaluation included diagnosis, patient management, and the cost-effectiveness of the workup.

  • Seventeen trainees completed 48 VIC modules, averaging CAN $227.52 and 68 virtual minutes per case.
  • While diagnostic accuracy was high (92.0%), management scores averaged 52.4%.

Conclusions:

  • VIC rheumatology modules are a positive and effective tool for teaching patient management and resource stewardship.
  • Future work includes expanding case variety, wider dissemination, and assessing long-term learning retention.
