Brain metastases (BM) distribution differs between brain regions. Supratentorial BM favor low perfusion areas, while infratentorial BM prefer high perfusion areas, suggesting distinct spreading mechanisms.

Conclusions:
- Distinct pathophysiological mechanisms likely underlie supratentorial and infratentorial BM development.
- Observed inverse perfusion patterns may relate to variations in vascular supply, hemodynamic needs, or angiogenic factor production.
