Isolated acalculia, the inability to perform calculations, can be a sign of stroke. This case highlights that stroke should be considered even without other neurological deficits.

  • Lesions in the left parietal cortex are the primary cause of acalculia.

Observation:

  • A 63-year-old male presented with a one-day history of isolated acalculia.
  • Neuroimaging revealed a left parietal lobe infarct and smaller infarcts in the left occipital lobe.

Findings:

  • The patient's isolated acalculia was attributed to a left parietal lobe infarct.
  • The absence of other neurological deficits in this case is noteworthy.
