Low hba1c; Is It Dapsone?

Summary

Dapsone use can falsely lower HbA1c levels due to hemolysis, impacting diabetes management. Discontinuing dapsone corrected the low HbA1c and normalized hemoglobin in a patient with diabetes.

Background:

  • A 51-year-old male with multiple comorbidities presented with dizziness and palpitations.
  • He had a history of diabetes and was on dapsone for dermatitis herpetiformis.

Observation:

  • The patient exhibited a persistently low glycated hemoglobin (HbA1c) level of 13 mmol/mol, inconsistent with his clinical presentation.
  • Investigations ruled out other causes for his symptoms and low HbA1c.

Findings:

  • Chronic dapsone therapy was identified as the cause of drug-induced hemolytic anemia.
  • Hemolysis led to a falsely reduced HbA1c measurement.

Implications:

  • Clinicians must consider drug-induced hemolysis as a cause of falsely low HbA1c, especially in patients on dapsone.
