New diagnostic models using frequency-domain optical coherence tomography (FD-OCT) show improved accuracy for detecting primary open angle glaucoma (POAG). The ONH-N model offers a simple and effective clinical tool for early POAG diagnosis.
Area of Science:
- Ophthalmology
- Medical Imaging
- Glaucoma Research
Background:
- Primary open angle glaucoma (POAG) is a leading cause of irreversible blindness.
- Early detection of POAG is crucial for timely intervention and vision preservation.
- Frequency-domain optical coherence tomography (FD-OCT) is a key imaging modality for assessing retinal nerve fiber layer (RNFL) thickness.
Purpose of the Study:
- To develop and evaluate a clinical diagnostic model for early POAG detection.
- To utilize normal probability charts derived from FD-OCT data.
- To compare the diagnostic performance of novel area partition strategies against traditional methods.
Main Methods:
- A cross-sectional study included 133 healthy subjects and 99 early POAG patients.
- Retinal nerve fiber layer (RNFL) thickness parameters were measured using RTVue-100 FD-OCT.
- Four diagnostic models were constructed based on optic nerve head (ONH) and RNFL3.45 parameters with traditional and new area partition strategies.
Main Results:
- New area partition models demonstrated superior diagnostic accuracy (AROC ONH-N: 0.990, RNFL3.45-N: 0.939) compared to traditional models (AROC ONH-T: 0.979, RNFL3.45-T: 0.881).
- No significant statistical difference was found between the AROCs of ONH-T, ONH-N, and RNFL3.45-N.
- The ONH-N model was identified as the simplest among the evaluated models.
Conclusions:
- The novel area partition models enhance diagnostic accuracy for early POAG detection.
- These improved models can significantly augment the clinical ability to diagnose early POAG.
- The ONH-N model, due to its simplicity, presents a promising tool for convenient clinical application.
