Background:

Personalized breast cancer screening economic evaluations traditionally assume full adherence.
Previous studies have not assessed the impact of nonadherence on personalized screening strategy selection.

To evaluate the economic effects of nonadherence on personalized breast cancer screening strategies.
To determine the most cost-effective personalized screening approach considering varying adherence levels.

Developed adherence scenarios based on literature.
Utilized a Markov microsimulation model to assess three personalized strategies under different adherence conditions.
Conducted probabilistic sensitivity analyses.

The absolute adherence rate is more critical than the risk-adherence relationship in determining cost-effectiveness.
Risk-stratified screening can be more cost-effective than routine screening below a $60,000 willingness-to-pay threshold.
