Background:

Systemic mastocytosis (SM) presents diverse bone manifestations, including osteosclerosis and osteoporosis.
Osteoporosis is a common and clinically significant complication of SM, particularly in adults.
This bone loss often leads to recurrent fragility fractures, especially vertebral fractures.

Purpose of the Study:

To review the bone manifestations in systemic mastocytosis.
To elucidate the mechanisms of bone loss in SM.
To discuss the rationale for bisphosphonate treatment in SM-related bone disease.

Main Methods:

Literature review of bone pathology in systemic mastocytosis.
Analysis of the role of osteoclasts and bone resorption.
Evaluation of signaling pathways (e.g., RANK-RANKL) and cytokines in bone metabolism.

Main Results:

Systemic mastocytosis can result in both osteosclerosis and osteoporosis.
Osteoporosis is a frequent and critical manifestation in adult patients with SM.
Increased osteoclast activity and bone resorption are the primary drivers of bone loss.

Conclusions:

The RANK-RANKL pathway and cytokines significantly influence bone resorption in SM.
Bisphosphonates, due to their anti-resorptive properties, represent a rational therapeutic choice for bone involvement in systemic mastocytosis.
