Background:

  • Standard neurological and neuropsychological exams may not identify all abnormalities following mild traumatic brain injury (mTBI).
  • Eye movement analysis offers a potentially more sensitive method for detecting neurological dysfunction post-mTBI.

Purpose of the Study:

  • To investigate the sensitivity of eye tracking assessments in detecting neurologic dysfunction after mild traumatic brain injury (mTBI).
  • To compare eye movement parameters between military personnel with persistent postconcussive symptoms and healthy controls.

Main Methods:

  • Eye tracking assessments were conducted on 71 military personnel with a history of mTBI and 75 healthy volunteers.
  • Participants performed visual tasks engaging saccadic and smooth pursuit systems, with eye movements sampled at 500 Hz.

Main Results:

  • mTBI participants exhibited shorter intersaccadic intervals and lower saccadic amplitudes during specific tasks.
  • Higher absolute fixation velocity and longer fixation/regression durations were observed in the mTBI group.
  • mTBI individuals demonstrated impaired smooth pursuit gains and altered fixation patterns during reading tasks.

Conclusions:

  • The saccadic and smooth pursuit eye movement systems show vulnerability following mild traumatic brain injury.
  • Eye tracking presents a promising, objective, and sensitive tool for assessing neurological damage after mTBI.
