Summary

Finite element analysis (FEA) of osteoporotic vertebral fractures (OVF) can be accurately predicted using multidetector computed tomography (MDCT) at reduced radiation doses. This allows for significant dose reduction while maintaining diagnostic accuracy for differentiating fracture groups.

Purpose of the Study:

  • To compare vertebral failure loads predicted by finite element (FE) analysis at reduced MDCT dose levels versus standard dose.
  • To evaluate if ultra-low dose FE analysis can differentiate patients with and without OVF.

Main Methods:

  • Retrospective study of 16 patients (8 with OVF, 8 controls).
  • MDCT images acquired at standard dose, then virtually reconstructed at half, quarter, and tenth dose levels.
  • FE analysis performed to determine vertebral failure load at L1-3.

Main Results:

  • Vertebral failure loads at standard, half, and quarter doses did not significantly differ.
  • Failure load differed significantly between fracture and control groups at standard, half, and quarter doses.
  • Tenth dose analysis showed no significant difference between groups and higher failure loads.

Conclusions:

  • MDCT-based FE analysis can predict vertebral failure load adequately with at least 75% dose reduction.
  • Reduced-dose MDCT is viable for non-invasive FE analysis in OVF assessment.
  • FE analysis at quarter-dose MDCT effectively differentiates fracture from control groups.
