  • To evaluate subgaleal bone flap preservation in the contralateral scalp.
  • To assess the safety and efficacy of this novel preservation technique.

Main Methods:

  • Multicenter retrospective study (2014-2016) of 23 severe traumatic brain injury patients.
  • Bone flaps preserved in subcutaneous scalp tissue.
  • Analysis of surgical site infection, bone resorption, and patient discomfort.

Main Results:

  • No surgical wound infections or bone resorption observed over 18 months.
  • No patient discomfort reported with scalp preservation.
  • Five patients died from systemic complications unrelated to bone flap preservation.

Conclusions:

  • Subgaleal bone flap preservation is a satisfactory alternative.
  • This technique is suitable for routine use in decompressive craniectomy patients.
