Narcolepsy is a chronic sleep disorder characterized by pervasive, uncontrolled sleepiness and other sleep disturbances. One of its hallmark symptoms is an abrupt transition to REM sleep upon falling asleep, which causes symptoms typically associated with this phase to occur unexpectedly during wakefulness. Summary

Status cataplecticus, a rare condition of frequent cataplexy, was suspected in a narcolepsy patient. However, evaluation revealed the spells were due to a somatoform disorder, not cataplexy.

Background:

Status cataplecticus is a rare, therapy-resistant condition involving multiple cataplexy episodes.
Narcolepsy type 1 is a chronic neurological disorder affecting sleep-wake cycles.

Observation:

A young adult with narcolepsy type 1 presented with severe symptoms mimicking status cataplecticus.
Initial presentation suggested a refractory cataplexy exacerbation.

Findings:

Neurological evaluation, polysomnography, and further history indicated the spells were not cataplexy.
The patient's symptoms were ultimately diagnosed as a somatoform disorder.

Implications:

This case underscores the need to consider psychosomatic factors in narcolepsy type 1.
It highlights the importance of reassessing diagnoses when cataplexy is refractory to treatment.
Considering somatoform disorders can lead to more accurate diagnosis and appropriate management in narcolepsy patients.
