Insights
Low-dose cyclosporine (CsA) or tacrolimus (Tac) combined with mycophenolate mofetil (MMF) shows promising renal remission and safety in children with proliferative lupus nephritis (PLN). This combination is a good option for patients with lupus nephritis experiencing relapse or suboptimal MMF therapy.
Area of Science:
- Pediatric Nephrology
- Immunosuppressive Therapy
- Autoimmune Diseases
Background:
- Proliferative lupus nephritis (PLN) is a severe complication of systemic lupus erythematosus in children.
- Maintenance therapy is crucial to prevent disease relapse and preserve renal function.
- Mycophenolate mofetil (MMF) is a common maintenance therapy, but some patients may relapse or show suboptimal response.
Purpose of the Study:
- To evaluate the efficacy and safety of adding low-dose cyclosporine (CsA) or tacrolimus (Tac) to MMF maintenance therapy in pediatric patients with PLN.
- To assess the rates of renal remission and clinical response over a 3-year period.
- To identify any severe adverse events associated with the combined immunosuppressive regimen.
Main Methods:
- Eleven children with PLN who relapsed on MMF maintenance therapy received add-on low-dose CsA or Tac.
- Renal remission was assessed at multiple time points: 3 and 6 months, and 1, 2, and 3 years post-intervention.
- Adverse events were systematically recorded throughout the study period.
Main Results:
- High clinical response rates were observed, reaching 100% at 6 months and sustained through 3 years.
- Complete renal remission rates varied, with a notable increase to 71.4% by the 3-year mark.
- No severe adverse events were reported in any of the participating patients.
Conclusions:
- The combination of low-dose CsA/Tac with MMF demonstrates a promising therapeutic effect for achieving renal remission in pediatric PLN.
- This regimen is associated with acceptable safety profiles, making it a viable option for managing relapsed or inadequately treated lupus nephritis.
- The findings support the use of this combination therapy in children with PLN requiring enhanced maintenance strategies.
Objective:
We aimed to evaluate the efficacy of low-dose cyclosporine (CsA) or tacrolimus (Tac) in children with proliferative lupus nephritis (PLN) during maintenance therapy.
Methods:
A low dose of CsA or Tac was added to 11 children who relapsed during mycophenolate mofetil (MMF) maintenance therapy. Renal remission was analyzed at 3 and 6 months, and at 1, 2, and 3 years after CsA/Tac addition. Adverse effects were recorded.
Results:
The clinical response rates were 81.9%, 100%, 90.0%, 100%, and 100% at 3 months, 6 months, 1 year, 2 years, and 3 years after CsA/Tac addition, respectively. Complete renal remission rates were 45.5%, 45.5%, 40.0%, 44.4%, and 71.4% at 3 months, 6 months, 1 year, 2 years, and 3 years after CsA/Tac addition, respectively. None of the patients had severe adverse events.
Conclusion:
Low-dose CsA/Tac combined with MMF shows a promising effect in renal remission with acceptable safety in children with PLN. Therefore, this combination would be a good choice for children with lupus nephritis who relapse or have suboptimal MMF maintenance therapy.

Conjugate Addition (1,4-Addition) vs Direct Addition (1,2-Addition)
Conjugate addition results in a thermodynamically stable product. The reaction retains the stronger C=O bond at the expense of the weaker C=C π bond. The process is slow as the β carbon is less electrophilic than the carbonyl carbon.
