Summary

Detectable interleukin-1β (IL-1β) serum levels at the start of an in vitro fertilization (IVF) cycle predict successful outcomes. Rising IL-1β levels during pregnancy indicate a viable implantation, while undetectable levels may signal pregnancy loss.

Background:

  • Maternal serum interleukin-1β (IL-1β) and interleukin-1 receptor antagonist (IL-1Ra) levels have been linked to in vitro fertilization (IVF) cycle outcomes in retrospective studies.
  • The role of the IL-1 cytokine family in implantation and pregnancy is well-established.

Purpose of the Study:

  • To prospectively investigate the correlation between serum IL-1β and IL-1Ra levels and IVF cycle outcomes.
  • To determine if IL-1β and IL-1Ra levels measured at various time points during an IVF cycle can predict pregnancy success.

Main Methods:

  • Prospective cohort study of 205 women undergoing IVF.
  • Serum samples collected before IVF, at hCG administration, and on days 24, 28, and 35 of the cycle.
  • Serum IL-1β and IL-1Ra analyzed using ELISA kits; cycle outcomes followed prospectively.

Main Results:

  • Detectable IL-1β levels at the start of the IVF cycle were associated with successful IVF outcomes (p=0.027).
  • Increasing IL-1β serum levels over time were observed in women with viable pregnancies (delivery or ongoing pregnancy), but not in those with pregnancy loss.
  • Undetectable IL-1β at the start of the cycle was linked to subsequent pregnancy loss in women with a positive pregnancy test (p=0.046).
  • No significant results were found for IL-1Ra serum levels.

Conclusions:

  • Prospective analysis confirms a positive association between detectable serum IL-1β at the start of an IVF cycle and successful IVF outcomes.
  • Rising IL-1β levels during pregnancy support its role in implantation and maintenance of gestation.
  • Serum IL-1β may serve as a predictive biomarker for IVF success and pregnancy viability.
